This is the most useful section. List everything that might be relevant, in order, with dates where you can — however small, accidental or embarrassing. Power events, a dropped or knocked keypad, forklift or impact knocks, spills, an axis forced by hand, a part that failed, an error someone saw. Nothing is too trivial: “someone jacked the Z-axis up off the table with a car jack” is exactly the kind of detail that solves a job.
